Submitting from inside Nevada
The RCCD records-request page links an electronic personal-history request and limits that route to submissions made inside Nevada. Follow that service’s instructions for your location.
Submitting from outside Nevada
Use the manual request linked on the same page. It is expressly required for out-of-state submissions. The personal-record packet is for the subject’s own history, not an unrestricted search of another person.
Check the manual packet before sending it
The currently linked DPS-006 manual instructions specify the following items and return-address rules.

Complete and sign the applicant form
Each person needs a separate completed form and packet. Unreadable or incomplete submissions may be returned.

Enclose an original fingerprint card
Use FD-258 with the required identifying details and impressions. The applicant must sign; a certified technician must take, date and sign the prints. Copies and previously processed cards are not accepted.

Supply exact payment
The packet lists $27 per applicant by money order or certified check payable to Nevada Department of Public Safety. Personal checks and cash are excluded. Use the packet’s mailing address at 333 West Nye Lane, Suite 100, Carson City, NV 89706.

Use a valid delivery address
The response cannot be forwarded. An address change requires a new DPS-006 form. The packet asks applicants to allow approximately 45 days after receipt; do not treat that as expedited service.
